Intake Process at Duluth Health Care Systems
Eligibility Criteria
To ensure that individuals receive the appropriate services and support, Duluth Health Care Systems has established the following eligibility criteria for our intake process
* Affiliation with a Regional Center: Applicants must be attached to a Regional Center to qualify for services.
* Documented Disability History: Applicants must have a documented disability history with the Regional Center.
* Age Range: Applicants should be between the ages of 18 and 99
* Willingness to Participate: Applicants must demonstrate the motivation and willingness to engage in the services offered by Duluth Health Care Systems.
Application Process
The intake process is streamlined to ensure efficiency and clarity:
- Referral Packet: Applicants will receive a referral packet from the Regional Center, outlining the necessary documentation and steps to begin the process.
- Assessment Review: Our team will conduct a thorough assessment to determine eligibility, identify areas of need, and outline the required support.
- Assessment Recommendation: After the review, we will send a recommendation to the Regional Center for further processing.
